Medical Exercise: What is it?

A Certified Medical Exercise Specialist (CMES) works with you to promote health outcomes and overall quality of life. A CMES has the qualifications to work in conjunction with your primary care physician or specialist depending on what outcomes that are desired.  It is most frequently used for patients working with Metabolic Disease, Post-Cardiac Rehab, Post-Physical Therapy, Obesity, Chronic Pulmonary Obstructive Disease, Asthma, Osteopenia/Osteoporosis, and more.

MiHealth is an Approved Silver & Fit location. Depending on your Medicare Advantage Plan, you may qualify for reduced cost sessions. To verify go to https://www.silverandfit.com/identity/registration

Medical Exercise: When would I need it?

A CMES is most useful when your physician or care provider has been encouraging you to begin an exercise program, but you may not have the exercise background to know how to safely begin one.  We have a conservative approach when working with medical patients with gradual improvements and goals

How to get started:
There are 3 forms required to begin an orientation at MiHealth. It is important to note that completing these forms is NOT a contract to sign up with MiHealth. The purpose of these forms is so we can preview any medical conditions and determine eligibility prior to beginning any exercise program.  If a clearance is required we will reach out to get clarification on the medical concern and send a copy of a physician's clearance to get your physician's recommendations for an exercise program.  This can all be done VIA Email/MyChart

New Membership Pricing and Info Form: This form is used to receive contact information, exercise history and goals, and schedule availability

Orientation Forms: This form is used to review our current terms and conditions ahead of time as we will review all 3 of these forms in person for your first orientation meeting

Medical and Exercise History: This form is used to review your medical history and any medical concerns you may have so they can be addressed prior to starting a program.
